Overview
The Rapid Rehousing Specialist works with participants, family members, and community supports to identify and secure short- and long-term housing resources. The Specialist coordinates services, benefits, training, and activities designed to foster recovery, maintain stable housing, and increase self-sufficiency in a manner consistent with MHP recovery principles.
Essential Job Duties
- Collaborate with participants to identify, develop, and access resources and supports to increase their success with self-sufficiency, community integration, and community inclusion.
- Support participants with program orientation, recovery plans, and acquiring stable housing (housing searches, coordinating with property owners, etc.); facilitate self-review of progress during each encounter.
- Obtain necessary information during intake and collaboratively document participant progress in MHP’s electronic health records system and participant files; verify data accuracy with participants.
- Comply with all established quality and regulatory standards, including housing inspections per Housing Quality Standards guidelines, verifying that participants’ housing meets Fair Market Rent standards, supervision, and training requirements.
- Collect and maintain monthly subsidies, rent receipts, and other relevant financial paperwork.
- Assist participants in managing their financial records related to rent, utilities, etc.
- Cultivate and maintain working relationships with external parties (property owners, referral sources, the county, etc.); attend professional meetings (coalitions, committees, etc.); collaborate with partner agencies for support and case management coordination.
- Assist participants with benefit acquisition (medical, mental health, substance abuse services), self-management of crisis situations, peer support groups, budgeting, and other topics as defined by participants.
- Support participants in the community during housing searches, events, visits, etc.
- Participate in recertification meetings to determine participants’ ongoing program eligibility; act as the property owner liaison to proactively manage recertification issues.
- Maintain compliance with all MHP policies and procedures; report any violations or conduct issues to management or the Director – Quality Improvement & Compliance and/or Human Resources.
- Perform other duties as assigned to fulfill MHP’s mission.
Requirements
- High School Diploma required; Associates degree preferred in a related field.
- Minimum of 2+ years of related work experience (including internships and volunteer work).
- Must possess a Housing Quality Standards certificate within 6 months of hire.
- Credentialed as a Certified Peer Specialist is preferred.
- Must possess a valid driver’s license.
- Must have basic computer skills (desktop computing, e-mail, timesheet management, electronic health records, internet research) and the ability to learn relevant software applications.
- Strong skillsets in oral and written communication, cultural competence, interpersonal relationships, attention to detail, customer service, time management, autonomy, problem-solving, and organization.
- Ability to work independently and complete assignments according to standard practices and prescribed routines.
- Must be able to work under pressure and meet deadlines while providing exemplary customer service.
- Certification in First Aid/CPR or the ability to obtain certification as required.
Physical Demands
- Sit at a desk with ergonomically appropriate equipment.
- Lift up to 15 lbs.
- Remain awake during the entire shift, including break times.
- 50% of the Specialist’s time is spent in community settings and outreach; must be able to travel within the community in inclement weather (by walking, driving agency vehicles, or using public transportation).
- Ability to carry computer equipment and ascend/descend stairs while carrying equipment.
