Case Managers provide services to participants living in the community with persistent mental health challenges, as well as to individuals with co-occurring conditions. After assessing participants' needs, the Case Manager helps obtain resources and services in the community in a partnership that focuses on recovery and the achievement of the participant's goals for community living.
The Case Manager Trainee is a full time position, Monday - Friday, 8:30 AM - 4:30 PM. This is an onsite work position. As a full time employee, you would be eligible for the following benefits:

What You'll Do
- Collaborate with participants to create self-directed goals that support their path to recovery and self-sufficiency.
- Support participants with educational, employment, and social-related goals; assist with accessing benefit/medical resources, mental health and substance use services, budgeting skills, and housing options in accordance with their support plan.
- Attend socialization activities, such as community events, recovery learning centers, 12-step meetings, etc.
- Collaboratively document participant progress in the electronic health records system.
- Perform other duties as assigned to fulfill MHP's mission.
Requirements
- A High School diploma with a minimum of 12 social science credits is required; a Bachelor's degree is preferred (majors such as Sociology, Social Work, Psychology, Gerontology, Anthropology, Political Science, History, Criminal Justice, Theology, Nursing, Counseling, or Education are ideal).
- Minimum of 2 years of experience in a behavioral health work environment is preferred.
- Must have or be able to obtain a BHTEN certification within one year of hire.
- Must possess a valid driver's license and have access to a car.
- If applicable, must qualify for and obtain a prison ID.
- Demonstrated knowledge of the local Mental Health System.
- Basic computer skills (desktop computing, e-mail, timesheet management, electronic health records, internet research) and the ability to learn relevant software applications (e.g., Microsoft Word, Excel).
- Ability to work independently and complete assignments within established routines and standard practices.
- Must be able to work under pressure, meet deadlines, and achieve billable unit requirements while maintaining a positive attitude and exemplary customer service.

Physical Demands
- Sit/stand at a desk with ergonomically appropriate equipment.
- Approximately 80% of a Targeted Case Manager's time is spent in community settings and/or outreach. Must be able to travel by walking, driving agency vehicles, or using public transportation depending on program needs.
