Case Managers provide services to participants living in the community with persistent mental health challenges, as well as to individuals with co-occurring conditions. After assessing participants' needs, the Targeted Case Manager assists in obtaining resources and services within a partnership that focuses on recovery and achieving community living goals.
The Case Manager is a full time position, Monday - Friday, 8:30 AM - 5:00 PM, with the opportunity to create a flexible schedule with manager approval. This hybrid role includes required in-person participant visits and office days with occasional work-from-home options. As a full time employee, you will be eligible for the following benefits:

What You'll Do
- Collaborate with participants to create self-directed goals supporting their recovery and self-sufficiency.
- Support participants with educational, employment, social goals, benefit/medical resources, mental health and substance use services, budgeting skills, and housing options in line with their support plan.
- Attend socialization activities, such as community events, recovery learning centers, 12-step meetings, etc.
- Document participant progress in the electronic health records system.
- Perform other duties as assigned to fulfill MHP's mission.
Requirements
- A High School diploma with a minimum of 12 social science credits required; a Bachelor’s degree is preferred (majors in Sociology, Social Work, Psychology, Gerontology, Anthropology, Political Science, History, Criminal Justice, Theology, Nursing, Counseling, or Education are preferred).
- Minimum of 1 year of case management experience in a fee-for-service environment.
- Must have or be able to obtain a BHTEN certification within one year of hire.
- Must possess a valid driver's license and have access to a car; automobile insurance and current state registration required.
- Demonstrated knowledge of the local Mental Health System.
- Basic computer skills including desktop computing, email, timesheet management, electronic health records, and internet research.
- Ability to work independently, meet deadlines, and achieve billable unit requirements while maintaining a positive attitude and exemplary customer service.
- Strong oral and written communication skills, interpersonal abilities, attention to detail, and effective problem-solving skills.
- Must be able to safeguard confidential or sensitive information.

Physical Demands
- Sit/stand at a desk with ergonomically appropriate equipment.
- Approximately 80% of the time is spent in community settings and outreach; must be able to travel by walking, driving agency vehicles, or using public transportation.
