Our TCM programs, ACCESS and SHARE, work to improve the quality of life for homeless Philadelphians with mental health conditions, people who require assistance in coordinating these services, and people who are being released from prisons and jails by assessing needs, defining desired outcomes, obtaining services, treatments and supports, and preventing and managing crises.
