Mental Health Partnerships (MHP) Delaware County Homeless Outreach Program provides comprehensive support to people experiencing homelessness who may or may not have mental illness, co-occurring disorders. Services include access to housing and other resources, care coordination, crisis intervention, and recovery services.

Mental Health Partnerships’ collaborative relationships with other organizations in Delaware County allows us to provide a warm hand-off to trusted partners for those not eligible for our services, and continue to follow up, as needed. Currently, we have one Recovery Learning Center in Delaware County. The Homeless Outreach Program will work closely with the RLC staff to ensure that individuals can receive meals, laundry, shower, and groups during the day if interested.
